St. Clare’s Homes – List of Services & Common Diagnoses

We are located in Jersey City, Elizabeth, and Neptune, NJ. However, children are accepted from throughout the state. St. Clare’s Homes are contracted to provide services through the Division of Child Protection & Permanency (DCP&P), and they were the first facilities licensed by the NJ Department of Health as Pediatric Transitional Community Homes. Below is a list of services we routinely provide and a list of common conditions in the children we have cared for over the past 27 years.

St. Clare’s Homes also have limited availability for emergency placements and placement of older children ages 13-18.

  • 24-Hour Supervision By Experienced Healthcare Professionals
  • All Staff is CPR Certified
  • Accommodation Of Family Visits And Family Team Meetings
  • Coordinate School Needs
  • Licensed Nursing Staff RN/LPN
  • Daily Observation/Assessment
  • Gastronomy Tube Feedings
  • Medication Administration
  • Facilitate All Medical Care
  • Monthly Case Conferences With Treatment Team
  • Nutritional Assessment Provided By a Registered Dietitian
  • Referral To Early Intervention Program
  • Respite Care
  • Wheelchair Accessible Homes
  • Coordination Of Family and Child Educational Needs
  • Discharge Planning
