Pediatric Transitional Homes for Children with Specialized Needs


The St. Clare's Homes are licensed by the NJ State Department of Health and Senior Services as Pediatric Community Transitional Homes - the first in the state to receive this distinction. 

St. Clare's Homes for Children offers transitional care to infants and children who have specialized medical needs. As a contracted partner with the Division of Child Protection and Permanency (DCP&P), St. Clare’s Homes are safe and provide loving environments for vulnerable children living at-risk in the community or who are boarded needlessly in a hospital setting. 


Through a holistic approach to housing and care, St. Clare’s provides children with medical, nutritional, developmental and social support at a critical period in their lives. Our team works with DCP&P caseworkers to ensure each child is ready for placement when permanent homes are identified. We also offer support to biological, foster and adoptive families through educational and training resources, and by provide comprehensive Respite Care.
